Physical Activity Readiness Questionnaire (PAR-Q)


If you are planning to become much more physically active than you are now, please read the questions below and answer each one honestly by ticking YES or NO. If you are between the ages of 15 and 69, the PAR-Q will tell you if you should check with your doctor before you start. If you are over 69 years of age, and you are not used to being very active, check with your doctor. 

If you answered YES to one or more questions, it is recommended that you talk with your doctor before starting any exercise. You may be able to do any activity you want – as long as you start slowly and build up gradually. Or, you may need to restrict your activities to those which are safe for you.


If you responded NO to the questions above please sign the waiver below.

To ensure the safety of clients and staff alike, we ask that you abide by the Jodie’s Aerobics rules regarding our fitness classes.

  • If you feel ill or have any symptoms, do not attend the class and you should follow the government guidelines.
  •  Arrive at least 5 minutes before the class start time to allow for social distancing measures while setting up. 
  •  Maintain at least two metres from all others when in and upon entering/exiting the building. 
  • Sanitise hands upon entry to the building.
  • Any equipment will be set up for you in your designated area. If you require different equipment, please inform the instructor before the class start time. 
  • Please leave your equipment in your designated spot after class for the instructor to sanitise again.
  • During class please stay within the floor markings of your designated spot unless advised to do otherwise. 

In signing this form, I confirm that I have read, understood and completed this questionnaire. I confirm that the above answers are true and accurate to the best of my knowledge and belief. Any questions I had were answered to my full satisfaction. I undertake to notify my fitness or health professional if any of the answers to the above questions change. I understand and am aware that strength, flexibility, and aerobic exercise, including the use of equipment, in the outdoors, or assigned training areas, are potentially hazardous activities. I am voluntarily participating in these activities and using equipment and facilities with the knowledge of the dangers involved.  I hereby agree to expressly assume and accept all and any risks of injury or death. I understand that Jodie’s Aerobics cannot accept responsibility for items lost during fitness classes and that I am responsible for all my belongings brought with me.
